To Sponsoring Union Officials:

We are very pleased that your organization has decided to nominate and sponsor an individual to attend the New York State AFL-CIO/Cornell Union Leadership Institute. This nomination form confirms your agreement to participate in the Institute. Your nominee agrees to attend and participate in the programs of the Institute. Your organization agrees to provide the necessary sponsorship and financial support for that individual’s participation. The NYS AFL-CIO/Cornell Union Leadership Institute agrees to keep you informed as to the participant’s progress, provide any additional assistance you request in keeping with the mission of the Institute, and involve you in the selection and supervision of the participant’s final Field Work project.

Financial Approval:
By submitting this form I agree on behalf of our organization that our organization will provide the financial support necessary for the nominee’s participation. This constitutes a tuition payment of $7,800 plus travel and lodging costs for each seminar minus any applicable financial aid.
