All students who were 15 at the time of their annual review have transition planning and services listed on their IEP. It is the students who turn 15 during the year who generally (75%) do not have transition addressed on their IEP.

Using Cleartrack, a list will be generated of all students who will turn 15 during the year we are planning for (generally the next school year). A list of these students will be given to the appropriate teacher so they know they need to write a transition IEP.
