Processes are established for sharing student information. (use local or statewide interagency agreements, the VESID/CBVH referral process and the confidentiality release process).

Have established protocols with VESID et al. by creating releases for our Program. System not used by all our schools yet. Process this year required that we get releases on a one-at-a-time basis. Not efficient.

We have approved releases which will be sent out in the "First Day Packets" (Fall '08) and returned to the school, to make the exchange of information process more streamlined, making for better referrals. [First Day Packets are already part of our 5 school protocols--this is simply an addition of the Release Form to the Packet.]

We will determine "if" the "First Day Packet w/ Release" strategy has worked as we start to make referrals for the students when they return in the Fall. The Releases either will, or will not, be in the student files. This can be documented by a running "tally" of whether or not the Releases were returned to the school.

If the "First Day Packet" Release is returned to the school (and therefore put in each student's file), we will have a streamlined system for getting each child "linked" to VESID (ie, application submitted). If the 1st day Packet does not5 work, we will try direct mailings with SASEs.
